Nature of Services
Rootera Wellness provides health and wellness coaching using nutrition, lifestyle education, Functional Diagnostic Nutrition® principles, and functional laboratory information when included in the client's program.
Services may include education and recommendations related to nutrition, hydration, sleep, exercise, stress reduction, supplementation, digestive wellness, and other lifestyle factors.
Rootera Wellness services are educational and supportive in nature and are not a substitute for medical care.
Scope of Practice
Rootera Wellness does not diagnose, treat, cure, prescribe for, or prevent disease and does not replace care from a licensed physician or other qualified healthcare professional.
The client understands that recommendations provided through Rootera Wellness are intended to support general health and wellness.
Clients are encouraged to maintain an appropriate relationship with their licensed healthcare providers and to discuss medical conditions, medications, pregnancy, laboratory findings, or significant changes in health with those providers.
Rootera Wellness does not advise clients to discontinue, modify, or begin prescription medications. Clients are encouraged to maintain a relationship with a qualified primary care physician and to consult them before making any significant changes to their healthcare regimen or supplement protocols.
Client Responsibilities
Successful coaching requires active participation.
The client agrees to:
Provide accurate and complete information regarding health history, medications, supplements, lifestyle, and other information relevant to coaching.
Communicate changes in health, medications, supplements, pregnancy status, or other circumstances that may affect recommendations.
Complete agreed-upon forms, assessments, laboratory testing, and other program activities when applicable.
Attend scheduled coaching sessions and communicate when appointments need to be changed.
Ask questions when recommendations or educational information are unclear.
Take responsibility for deciding whether and how to implement wellness recommendations.
Continue seeking appropriate medical care when necessary.
The client understands that outcomes vary and that no specific health outcome is guaranteed.
5. Functional Laboratory Testing
Some Rootera Wellness programs include or offer optional functional laboratory testing.
When testing is selected, Rootera Wellness may assist with ordering tests, reviewing results, providing education regarding findings, and using those findings to inform wellness recommendations within the practitioner's scope. Rootera Wellness cannot diagnose, treat, or cure any disease. Functional laboratory testing is meant for educational use only.
Functional testing is not intended to replace diagnostic testing or evaluation performed by a licensed medical provider.
The client is responsible for following all laboratory preparation, collection, storage, and shipping instructions.
Laboratory fees may be separate from coaching fees and are subject to the applicable laboratory's policies.
Clients are encouraged to maintain a relationship with a qualified primary care physician and to consult them before making any significant changes to their healthcare regimen or supplement protocols.

Supplements
Supplement recommendations may be provided when appropriate as part of a client's wellness plan.
The client agrees to disclose current medications, supplements, allergies, known sensitivities, pregnancy or breastfeeding status, and other relevant health information before implementing supplement recommendations.
Clients should discuss potential medication interactions, contraindications, medical conditions, pregnancy, breastfeeding, surgery, or other medical concerns with an appropriate licensed healthcare professional.
The client understands that supplement recommendations may be modified as the program progresses and that more supplementation is not necessarily better.
Payment
Payment is required in full before services begin unless the client has entered into a written payment plan approved by Rootera Wellness.
Clients enrolled in an approved payment plan must remain current on all scheduled payments in order to continue receiving services.
Failure to make an agreed-upon payment may result in suspension of services until the account is brought current.
Laboratory fees, supplements, and other optional products or services may be charged separately unless specifically stated otherwise.
Refunds
All purchases are subject to the Rootera Wellness Payment, Cancellation & Refund Policy.
Program purchases are non-refundable. Choosing to discontinue participation, failing to schedule or attend available sessions, or deciding not to implement recommendations does not create an entitlement to a full or partial refund.
The client acknowledges that purchasing a program reserves practitioner time, availability, preparation, resources, and support.
Scheduling & Cancellations
Clients should provide at least 24 hours' notice when rescheduling or canceling an appointment.
Appointments missed or canceled with less than 24 hours' notice may be considered used except when otherwise determined by Rootera Wellness.
Programs are intended to be completed within the timeframe described at enrollment. Any extension of the program period is at the discretion of Rootera Wellness.
